Requests

Requests for a copy of the report verifying a crime or listing lost property can be obtained by clicking on this hyperlink and properly completing the necessary captions on a VERIFICATION OF CRIME / LOST PROPERTY [PD 542-061] form.

Print the form and mail it along with a self addressed-stamped envelope with a check or money order for $15.00 payable to Police Department City of New York.

The AIDED UNIT, a sub-unit of the Central Records Division processes Aided Reports as well as requests for copies of Aided Reports. Requests for a copy of an Aided Report can be obtained by clicking on this hyperlink and properly completing the necessary captions on a REQUEST FOR COPY OF AIDED [PD304-161] form.

Print the form and mail it along with a self-addressed stamped envelope to the New York City Police Department, Criminal Records Section (Aided Unit).



Collision Records

Police Collision Records are retained at the Precinct in which the accident occurred for thirty (30) days before being forwarded to the Commissioner of Motor Vehicles. Motorists may request a copy of their collision record at the Precinct of occurrence within thirty (30) days of the report by completing a REQUEST FOR COPY OF COLLISION RECORD [PD301-165].

After thirty (30) days from the date of the Police Collision Record all requests must be made to the New York State Department of Motor Vehicles on NYS DMV form MV-198C. The NYS DMV requires a twenty dollar ($20.00) fee for this request.
